GD0 field 32, positions 84 - 91, is the date CPM started. This field tells the carrier if the patient is using a cardio pulmonary activity monitor. If they are then the date they started using it is filled in here. This can be in either yyyymmdd or mmddyyyy format, depending on the requirements of the carrier.
GD0 field 33, position 92, is the intractable lymphedema indicator. This field tells the carrier if the patient has intractable lymphedema of the extremities. If so, this field is filled with a Y. If not, then the field is filled with an N.
GD0 fields 34, 35 and 36, positions 93 - 125, is the first name, last name and middle initial of the doctor who prescribed the durable medical equipment for this patient. This is a mandatory field and if the CMN is sent, this field must be filled in or the claim will be denied.
GD0 field 37, positions 126 - 140, is the ordering provider ID number. This is the ID number of the doctor who prescribed the durable medical equipment for the patient. This field is also mandatory and must be filled in if this CMN is transmitted.
GD0 field 38, positions 141 - 150, is the ordering provider phone number field. This field transmits the phone number of the doctor who prescribed the durable medical equipment for the patient. This is important so that the carrier can contact the doctor in case there are questions about the CMN or the equipment.
GD0 field 39, positions 151 - 158, is the date of the CMN. Many people have a problem with this date as it can change. If this CMN is the initial certification, as designated by that field, then the original date of the CMN is transmitted here. If however, this is a recertification of a piece of equipment that was prescribed a while ago, then the recertification date needs to be transmitted here. Many claims are denied because the wrong date is transmitted in this field. The date can go over as yyyymmdd or mmddyyyy, depending on the requirements of the carrier.
GD0 field 40, position 159, is the certification on file indicator. All CMNs, even though they can be transmitted electronically, have to have a paper file copy. This is required by law. So it may seem strange that this field would even exist. The reason it does is that the claim can be electronically transmitted before the actual CMN is on file. This allows for speedy claims processing. However, if audited and no CMN is found to be on file for this patient, serious repercussions will be felt by all parties involved. This field is either filled with a Y or N.
