This record identifies the patient the services are being billed for. We're going to go over the individual specifications for this record so you will know exactly what is required.
CA0 field 1, positions 1 - 3, is the record type, which in this case is the CA0 record. This field must be filled in with CA0 or the whole claim will be rejected.
CA0 field 2, positions 4 - 5, is reserved for future use. The funny thing is, this field has been reserved for future use forever. Dollars to donuts it stays that way.
CA0 field 3, positions 6 - 22, is the patient ID. This is an ID number that is assigned to each patient by the insurance carrier and it better be exact to the digit or the claim will be denied.
CA0 fields 4, 5 and 6, positions 23 - 55 are the patients last name, first name and middle initials. These must be the names as listed on the birth certificate. No nicknames.
CA0 field 7, positions 56 - 58, is the patient's generation. If you're wondering what this means, it is in the case of a patient who has a name like Charles Johnson III. This is a required field if this is part of the patient's legal name.
CA0 field 8, positions 59 - 66, is the patient's date of birth. This is a tricky field because each carrier has different formats. Some require mmddyyyy and some yyyymmdd. So make sure you check with your carrier on what format they require.

CA0 field 17, position 170, is the patients marital status. This is a one character code that is either M, S, D, or W for married, single, divorced or widow/widower.
CA0 field 18, position 171, is the patient's student status. This is a one character code for a patient who is attending school and there is a list of valid codes in the handbook.
CA0 field 19, position 172, is the patient's employment status. Common codes are E for employed or U for unemployed. There are however other codes for things like disabled or on disability.
